"How do I get to there?"

"Do you take my insurance?"
We accept most major insurance plans. We encourage our patients to take an active role in their health care by having knowledge of their insurance coverage. If there are questions regarding insurance plans, patients can contact our office or their insurance carrier for assistance or further information. Patients must have valid proof of insurance at each office visit to keep medical care costs to a minimum and allow for proper claims to be filed for the services rendered. Insurance co-payments for office services are due at the time they are rendered.
While we make every effort to make your visit with us as smooth as possible, it is your responsibility to check with your insurance company to see if we are participating. Insurances are constantly changing and new rules and lists of participating physicians change almost daily. We are a specialty office and participate with most insurances. Some require a referral from your insurance carrier or primary care physician.
"Do I need a referral?"
Check with your insurance company. If authorization or referral from a primary care provider is required by your insurance plan we request that you advise our office of this requirement at the time of appointment scheduling, and assist us in obtaining the appropriate contact information for the authorization or referral.
If you have a primary care physician, you should talk with the doctor. If you belong to an HMO, Clinic Plan, or other managed care network you must bring a referral with you at the time of your visit. Our doctors do not belong to any managed care networks.
"I don't have a referral for today. Can I still be seen?"
Unfortunately, no. Without a referral from your primary care physician we cannot see you for your scheduled appointment and you will have to reschedule. Patients are responsible for obtaining the proper referral from their primary care physicians before their appointment.
"Where do I get my referral?"
You obtain the referral form from your primary care physician's office. You should have it in hand when you come for your visit.
"What should the referral have on it?"
The referral should include authorization for the office visit, the number of visits authorized, and authorization for any x-rays the doctor may need. If there are additional procedures being done, they should be authorized as well, such as casts and fracture care, or injections. The doctors CANNOT perform procedures that are not authorized by the primary care physician.
"The doctor's office told me they faxed my referral. What am I supposed to do now?"
Prior to your visit, give us a call. We will gladly let you know if your referral has been faxed. However it has been our experience that most faxes come too late for the appointment. It is best to have your referral in hand when you come for your appointment. If the referral is not available, then we will reschedule your visit to the first available appointment.
"I have a question about a bill I received. Whom do I speak with?"
Please call our billing department toll-free at 1-313-277-6700. They will be happy to answer any of your billing questions.
Fees and Payment
We make every effort to decrease the cost of your medical care. Therefore, we request payment for all office services at the time they are rendered unless prior arrangements have been made. For your convenience we accept cash, checks, Visa, MasterCard, and Discover. If we are a participating provider with your insurance company we will bill them. If you have no insurance coverage, payment for office services is expected at the time they are rendered unless prior arrangements have been made. Payment for services is the patient’s responsibility, and we will help in any way we can to assist you in handling claims.
"I have some insurance forms that the doctor needs to fill out for me. What do I do?"
Please give the forms to be filled out to the person at either the reception or appointment desk.
There is a charge for each form completed. Forms needing to be mailed should have self-addressed stamped envelopes with them, otherwise, you may pick the forms up when they are ready. We are unable to fax forms.
"What do I do if I need a refill on my prescription?"
Prescription refills should be called into the office during regular business hours.

3:00pm, Monday through Friday when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances. If doctor approved, they are called into your pharmacist within 24 hours.
All requests will be addressed within 48 hours.
"If I need x-rays, can you do them at the time of my appointment?"
Yes, we have state-of-the-art digital x-ray facilities here in the office.
"I am an Auto Accident patient. What do I need to do to see the doctor?"
To make an appointment as a result of an auto accident, you must have a letter from the Auto Insurance Carrier, stating that there is an open claim with the address for billing, contact person & phone numbers, and any claim number if issued.
Work Status and Disability Documents
The doctors and staff recognize that some of our patient’s orthopaedic concerns may require work or school absences and/or result in lost time. We will be happy to complete our standard office form free of charge documenting your absence or lost time if it is consistent with your current diagnosis. We also understand that your employer may require additional documentation for your work absence. Due to the time required to complete these additional forms, they will be completed by our office staff for a nominal fee, and will be ready within 5-7 business days of the day they are received.
"I am a Worker's Compensation patient. What do I need to do to see the doctor?"
To make an appointment with a work related injury, the visit must be authorized by the Workman's Compensation Carrier for your place of employment. They should give you a referral authorizing treatment, the address for billing, contact person & phone numbers, and any claim number if issued. This includes seeking second opinions on your own for work related injuries!
"How soon can I get in?"
We are a specialty practice and make every effort to get your appointment scheduled as soon as possible. Depending on the doctor appointment times vary between 2 to 4 weeks. Emergency appointments are scheduled as soon as possible according to the severity of the injury.
